What you'll need
Two things, and about a minute. We ask because these are your medical records — we have to be sure it's really you.
Your name and date of birth
Exactly as you gave them at your visit. If you've changed your name since, use the name we had then.
Your phone
We text a 6-digit code to the mobile number already on your account — so make sure you have it handy. We never send the code to a number you type in.
No mobile number on file, or it's changed? Our records team can verify you another way, or our front desk can set you up with an access card at your next visit.
